Background InformationChromogranin A (a protein of 439 amino acids which is encoded on chromosome 14) is present in neuroendocrine cells throughout the body, including the neuroendocrine cells of the large and small intestine, adrenal medulla and pancreatic islets. It is an excellent marker for carcinoid tumors, phenochromocytomas, paragangliomas, and other neuroendocrine tumors. Coexpression of chromogranin A and neuron specific enolase (NSE) is common in neuroendocrine neoplasms.

SpecificityChromogranin, an acidic protein having a molecular weight of about 68 kDa abides in the polypeptide-hormone-containing secretory granules of human endocrine tissues. Anti-chromogranin A can be used to stain most normal and neoplastic endocrine tissues containing secretory granules (exceptions include cells from the hypothalamus and posterior pituitary, which are known to contain secretory granules). The amount of staining is usually proportional to the number of secretory granules present (1).

It is found in secretory vesicles of neurons and endocrine cells. This gene product is a precursor to three biologically active peptides; vasostatin, pancreastatin, and parastatin. These peptides act as autocrine or paracrine negative modulators of the neuroendocrine system. Other peptides, including chromostatin, beta-granin, WE-14 and GE-25, are also derived from the full-length protein. However, biological activities for these molecules have not been shown.
