      Background Information3-monoglucuronyl glycyrrhetinic acid (3-MGA) is one of the metabolites of glycyrrhizin (GL) contained in licorice. It is reported to cause licorice-induced pseudoaldosteronism. It acts as an inhibitor of 11 beta-hydroxysteroid dehydrogenase, which allows cortisol to exert its full mineralocorticoid effects. Orally ingested GL is metabolized to 3-MGA and glycyrrhetinic acid (GA) by intestinal bacteria and subsequently 3-MGA is absorbed into the blood stream. Only a small fraction of absorbed GA is transformed to 3MGA in the liver by UDP-glucuronyltransferase. Circulating GA is metabolized in the liver to 3-MGA, which is excreted into the bile via multi-drug resistance protein 2 (Mrp2). However, if Mrp2 function is damaged then 3-MGA is secreted from the liver into the circulation and excreted into the urine via organic anion transporters. It is reported that circulating GA cannot be excreted into the urine since GA binds highly to serum albumin and thus does not pass through glomerular filtration and is not a substrate of transporters expressed on tubular epithelial cells. 3-MGA has a long plasma half-life and can cause hypokalemia. Clone 6H is suitable for detection of 3-MGA by ELISA and exhibits only minimal cross-reactivity with GA and GL. (Ref.: Kato, H., et al. (1995). J. Clin. Endocrinol.
