Nav1.4 is a multi-pass membrane protein that is a component of a voltage-sensitive sodium channel complex, which consists of a pore-forming alpha subunit and one or more regulatory beta subunits. This pore-forming subunit allows Na+ ions to pass in accordance with their electrochemical gradient. It is shown to be essential for normal muscle fiber excitability and for normal muscle contraction and relaxation cycles. It also maintains constant muscle strength in the presence of fluctuating K+ levels. It alternates between resting, activated, and inactivated states. Its channel activity is regulated by the ancillary beta that enhances the presence of the pore-forming alpha subunit at the cell surface. Its interaction with beta subunit is required for rapid channel inactivation and rapid recovery after inactivation and prevents decrease of channel activity in response to repetitive, high-frequency depolarizations. Nav1.4 channel activity is inhibited by tetrodotoxin. Nav1.4 undergoes phosphorylation at serine 1321 by protein kinase C in a highly conserved cytoplasmic loop that leads to inactivation of the sodium channel and reduces peak sodium currents. It contains 4 internal repeats, each with 5 hydrophobic segments (S1, S2, S3, S5, S6) and one positively charged segment (S4). Segment S4 is suggested to be the voltage-sensor and is characterized by a series of positively charged amino acids at every third position. (Ref.: Webb, J., et al. (2009).
